Family History: Diabetes (Type 02), Heart disease.


Medical report- 
Symptoms: Acute headache, disorientation, insomnia, hallucinations.
Notes:
Patient complained of a sense of unease, along with slight disorientation due to an unknown cause. Symptoms
reported to have appeared twelve days prior to appointment, and have persisted ever since primary development.
Four days prior to appointment however, patient has noted certaininstances of insomnia, and has also reported
hearing strange sounds, along with various other sensory anomalies. Some of these instances have been described
as seeing figures or apparitions which on second glance have not been real, feeling a presence nearby when there is
none, and other various hallucinations.

As treatment, I have prescribed Mr Akers with 25mg of Nitrazepam, and have recommended he keep his mind relaxed and
avoid stressful situations wherever possible. If symptoms still persist in 1-2 weeks, I have advised he schedule
another appointment, since this condition has had a recent surge of cases recently within the last month and we
can take further action in regards to his diagnosis.
